services offered determine which of our programs is appropriate for

training, exercise, aerobics, yoga, pilates, or circuit training activities
under the direct supervision of a fitness trainer. |

Designed for health/fitness clubs with annual sales of less than
$500,000 offering programs and services that may include personal
training, aerobics, yoga, pilates, free weights, resistance and cardio
machines for their members. All Eligible Operations Must Be Fully
Staffed During All Open Hours. |

health and fitness programs and services including 24hr key card
access facilities. Annual revenue must be $2M or less. |

services including, but not limited to: pro/sport shops, snack/juice bars,
restaurant/lounge, spa services, tanning, babysitting/child care,
