Our Agency Management System includes ACORD 36 (2007/01) -- Agent/Broker of Record Change which does the trick.

Here's one I've been using for years. I just type in the client info at the top rather than them transferring to letterhead. Fill in the other pertain-ant info and email it. The only time that hasn't work is if a company specifically requests client letterhead but that doesn't happen very often. The wording is good for transfer directly to letterhead and the client can cut and paste. And of course there's the Acord 36 as mentioned above that I use every so often. Hope this is helpful.

Hey Michael - you are welcome. Do a google search for Acord 36. In fact any time you need an acord form just google it by Acord (number).
If you have adobe reader you can find it as a fillable form and use it that way. But you will probably not be able to save the typed info. If you have adobe acrobat you can use it AND save the information that you have entered. Generally I've found that you can't save the fillable forms unless you have the paid version of adobe.
I've been using this document (attached above) for P&C since 1977 and have not had any issues with it. It's not just for Life & Health. And I've never been asked to have it or any BofR notarized.

Hello Michael,
Depending on the Product involved will kind of dictate which AOR form or Letter that will satisfy the Insurance Carrier and / or MGA.
In the realm of P&C, the Acord forms are the most accepted format, and as mentioned above the Acord 36 for an AOR. There are carriers that insist on utilizing their own created and worded forms as well.
With Life & Health the letters shown above on a clients letterhead will usually suffice.
Also is some cases I will also have these forms and letters Notarized, as I am and my co-workers are notaries.
It can help move the process move along a bit quicker.

Hi Michael!

My only word of advice on this is to check with the carrier to see if they have any requirements. The vast majority of companies will accept Acord forms, but our office does have one carrier that requires the letters to be typed up on the insured's letterhead. They will actually decline to process with anything less. We have another carrier that will accept AORs, but does require a copy of a newly completed app be sent in with it. This is to verify that the current agent knows exactly what coverages are included on the policy.
