How To Do Business with GMI

If you are a licensed retail insurance agent interested in becoming an appointed producer for GMI Insurance you will need to complete our Producer Application and Agreement. This agreement and the required items below must be provided when you are ready to bind your first account with us.

In addition to the completed application and agreement you must provide the following items:
  1. A copy of your E&O Declarations Page or Certificate of Insurance indicating coverage is in force. You must carry a minimum $1,000,000 liability limit.
  2. A copy of your key personnel's Resident and Non-Resident Licenses for states that you plan on submitting business from.
  3. A copy of your agency's Resident and Non-Resident Licenses for states that you plan on submitting business from.
  4. A signed copy of current IRS W-9 Form
