Submission Requirements for Axiom's Workers' Compensation Insurance program:

 •Signed ACORDs Workers Compensation (130) & Supplemental (where applicable)
 •Five Years Currently Valued Loss Runs
 •Current Experience Modification Worksheet
 •Suggested target pricing or expiring premium
