Premiums start at about $4,000. To seek proposals, we require the attached or a similar application to be completed.  If the advisor is registered with the SEC, we'll need a copy of Part 1 and 2 of the ADV form they file annually.  Financials, performance statistics, a prospectus for each fund they manage, client agreements, and resumes will also be very helpful.
