Many Agency Management Systems come with Templates to design proposals. In the end, you have to customize the proposal to meet your needs.
I have one that I've used for many years. I try to keep it up to date and as easy to understand as possible. You can google insurance agency proposals for lots of examples.

Probably the best in the industry is put out by Al Diamond. 6th Sense Proposals. Full Color - But is time consuming - so when you present you better be ready to talk for awhile.
