Submission Requirements

◾	Completed ACORD applications including: Radius of operations, garaging addresses, if there are additional named insured what is the relationship & are the operations the same?
◾	MVRs for all drivers along with driver schedule
◾	4 years currently valued loss runs (account is not rated without loss runs)
◾	Detailed descriptions of operations including what is being hauled and if there is any hazardous material
◾	GMI Supplemental Application (this answers a lot of other questions that are required) and is required to bind
◾	Need by date (date you need our quote in-hand)
◾	What is the expiring premium or renewal premium
◾	Does the insured require a TPL? A copy of the Dec Page will be needed for our file
◾	If MCS 90 is required, what is the limit?
◾	Stated values for any tanks, vacuum trucks, waste trucks or any modified vehicles
