offering programs and services that may include personal training,
aerobics, yoga, pilates, free weights, resistance and cardio machines for

and fitness programs and services including 24hr key card access facilities.
Annual revenue must be $2M or less |

including, but not limited to: pro/sport shops, snack/juice bars,
restaurant/lounge, spa services, tanning, babysitting/child care, massage

Upon binding the first account, agencies and individual producers are required to provide proper licensing documentation and errors & omissions coverage verification.
