Underwriting Requirements and Minimum Premiums

To submit an account for consideration, please provide the following: • Completed Workers Compensation Application with FEIN
• Five years of premium and payroll history
• Five years of currently valued loss runs
• Details on losses over $25,000
• New and expiring Experience Rating Worksheets
• Number of employees per class code
Minimum premium requirements:
  • $10,000 for most classes
  • $20,000 minimum for sawmills
We welcome both new and renewal business and can assist you with accounts that have prior loss challenges or are transitioning from assigned risk pools.
