What to Bring? Checklist for 2013 Tax Returns (You will need SOME or ALL of these items) 1. 2012 Tax Return 2. Income information for 2013: a. W-2 (s) from employer (s) b. Bank and other interest income (Bring original statements of 1099's ) c. Stock dividend statements and broker statements d. Broker statements for both the original stock purchase and sale of stock e. U. S. Bond and Treasury bill income statements f. Municipal bond income (both taxable and non-taxable) g. Unemployment compensation (fully taxable) h. Social Security and Railroad Retirement statements I. Pension statements (1099-R) j. IRA distributions, lump sum distributions and all other 1099 income k. Self-employment income AND expense records (including 1099's ) l. Gambling, scholarship and other miscellaneous income records m. All ROTH statements (new or converted) 3. All stockbroker transactions statements and final year end statements 4. Rental income and expenses 5. End-of-year IRA/ROTH IRA statements 6. Sale of Personal Residence (consult tax preparer if owned and occupied less than 2 years) 7. Dependents: Original Social Security Cards showing registration of name per Social Security Administration. Also Dependent's income information. 8. Adjustments to income: a. IRA (Individual Retirement Account) / SEP (Self-Employed Pension) Plans b. Self-employed Medical Insurance Premiums paid c. Penalty for early cash-in of bank certificates d. Alimony paid including Social Security number of recipient e. Student loan interest paid f. Educational expenses paid for self, spouse and dependents 9. Completed ITEMIZED DEDUCTIONS CHECKLIST-or equivalent (over) 10. Homestead, Rebate or Rent Credit (CRP) a. Taxable and non-taxable income (include welfare, worker's comp etc) b. Rent statement (CRP): This form should be provided to you from your landlord by January 31st. It must be completely filled out and signed by the landlord. The landlord could be penalized for not providing you with one. This penalty may be as much as $100.00. c Homeowners: please provide both the 2013 and 2014 property tax statements (if available). 11. Miscellaneous: Child and Dependent care expenses (including provider?' name, address and SS #) Death certificate (copy) of deceased taxpayer, Estimated Federal/State taxes paid
