AGENCY LICENSING

Thank you for your interest in working with Vintage Underwriters.

  • The Owner / Principal of the agency needs to review, complete, and sign the Producer Profile & Agreement. (All pages must be sent)
  • A copy of the insurance license(s) for each licensed agent / broker in your office as required by your State Insurance department. This includes Property and Casualty License as well as any other license required by your state to place coverage with a Surplus Lines Company.
  • Contact name for each licensed agent / broker in your office as well as their email address.
  • Copy of your Errors and Omissions Declarations page showing that E&O coverage is in place with minimum limits of $1,000,000
  • When completed, email to me at [email protected]         

Note: If you are signing up under a business name, that entity must be licensed with the Department of Insurance otherwise you must sign the agreement in your personal name.
