CLAIMS PROCEDURE GUIDE
Should a claim occur, it is imperative that it be reported to our office immediately. Call (Agency Phone Number) and your claim report will be taken and processed quickly. Following are a few guidelines to assist you in giving your report.
Sample claim report forms have been provided to you, indicating the information needed to settle a claim for Property, Liability, General Liability, Automobile, Cargo, and Miscellaneous losses.
Here's the procedure:
1. Obtain all the information requested on the proper form concerning the claim being made. (You do not have to complete the policy information.)
2. Call us at the above number, and we will take your report or advise you to mail it to us, depending upon the type or severity of the loss.
Reminders:
- Property: Protect your property from further loss.
- Crime: Report the crime to the police immediately, and include information about inventory loss (including serial numbers, color, age, and replacement cost).
- Liability: Call the paramedics if medical attention is needed. Obtain the names of all parties involved and witnesses. Do not admit fault at any time.
- Automobile: Call the police and make a report if there is bodily injury and/or serious property damage. Obtain the name of the officer and the station to which the accident is being reported. Do not admit fault. Exchange information with the other driver. Do not authorize any repairs to the other vehicle nor to your own vehicle until an adjuster has inspected the vehicles and approved repairs.
- Summons and Complaint: If you are served with a summons, write the date and by whom it was served on a note; attach it to the summons and call us. It is urgent that we receive the summons and forward it to the company. We will instruct you at that time.
- Workers Compensation: Complete the Employers' First Report of Injury form carefully. This form must be mailed to the insurance company within ( ) days after the employee injury. The address of the insurance company is at the top of the form.
Make two photocopies of the completed form; send one to us, and keep one for your file. We will keep it on file, should there be a question or problem in connection with the claim in the future. Be sure all employees are told to advise the clinic or doctor that this is a work-incurred injury. And if you have any questions, please don't hesitate to call us.
