 •  Pizza delivery operations
 •  Triple trailers
 •  Regulated medical waste or biohazardous waste

 •  Student drivers
 •  Any risk that allows nonemployee passengers

 

Requirements:
•  Loss information for current and prior three years
•  Complete VINs on all power units
•  Complete drivers’ list
•  All drivers must have acceptable MVRs.
